The seven academic and clinical organizations defined key questions and then used these questions to frame focus groups, surveys, and commissioned papers leading to a national summit on December 6-7, 2020. All in all, over 3,500 people participated in the process and a permanent website curates the products.1 The papers in this issue are the products of the process. They are diverse and passionate, like the specialty and the people who created them, but what are the big messages going forward?
